Community Care Hubs are community-driven, regional networks that help people connect to social and health services that match their unique needs. Community Care Hubs operate under their regional Accountable Community of Health (ACH) to support the needs of individuals and families. When people receive support—such as housing, food, mental health care, or childcare—their health and the health of their communities improves.
Community Care Hubs help improve community health by:
• Connecting people to social and health services,
• Building capacity for community organizations and workers, and
• Connecting systems through referral coordination.
